How To Apply
- Contact HMS and speak with our social worker.
- Arrange to visit and tour the school.
- Talk with your school district about your interest.
- Gather and submit your child’s records to HMS. This information should include the following:
- Re-evaluation Report (RR)
- Current IEP (Individual Education Program)
- Medical summaries
- If available—hearing, vision and psychological evaluations
